List of mandatory documents required at the time of counseling and admission

Candidates must bring all original documents along with two sets of attested photocopies at the time of counseling and admission. Failure to produce required documents may lead to cancellation of admission.

NEET (UG) Admit Card
NEET (UG) Score Card / Mark Sheet
H.S.C. / 12th Examination Mark Sheet
S.S.C. / 10th Passing Certificate (Date of Birth proof)
College Leaving Certificate

Identity & Nationality Documents

Nationality Certificate issued by District / Metropolitan Magistrate
OR Valid Indian Passport
OR School Leaving Certificate indicating nationality as Indian
Domicile Certificate
Medical Fitness Certificate

Category / Reservation Documents (If Applicable)

Caste Certificate
Caste Validity Certificate
Non-Creamy Layer Certificate
Gap Affidavit

PWD Candidates

Medical Fitness Certificate issued by an Authorized Medical Board
